Your First Visit
When you arrive for your first appointment, we’ll go over your new patient paperwork and give you a tour of our office. You’ll meet the doctor and team, then receive a comprehensive examination of your smile, including X-rays, if needed, and a dental cleaning.
Dr. Loftus will explain the results of your exam and discuss treatment options and next steps.
New Patient Forms
We’ve made our new patient forms available to you as Adobe Acrobat files. For your convenience, we recommend printing and completing these forms at home.
Please remember to bring your completed forms with you to your first appointment. If you have any problem getting these files to print properly, call our office and we’ll gladly send or fax them to you.
Payment Methods, Insurance, & Financing
Loftus Family Dentistry accepts cash, checks, and all major credit cards as payment for services rendered.
Our office takes the hassle out of dental insurance. Dr. Loftus is a provider for Delta Dental, and we file out-of-network claims for other providers, as well. Just present your insurance card at your first appointment and, if your insurance carrier allows you to choose your dentist, we’ll take care of the rest. Expect to pay only your co-pay or deductible at your visit. If you have any questions, don’t hesitate to call our office for more information.
